The Ohio State University College of Public Health Employee of the Year Nomination Form

Purpose

The Employee of the Year Program is intended to provide non-faculty employees of the College of Public Health with a formal method for acknowledging the contributions of individuals who are recognized by their colleagues as outstanding staff members.

Eligibility Requirements

In order to be eligible, employees must meet the following criteria:

  1. Must have been employed in the College of Public Health for at least 3 years.
  2. Must have at least an “Above Average” performance evaluation during the past two rating periods.
  3. Must have “Above Average” attendance record.
  4. Cannot have been selected as Employee of the Year during five previous years.

Criteria for Nomination

Nominees for Employee of the Year should exhibit the following characteristics:

  1. Exhibits excellent relations with co-workers and the public through their actions and attitudes.
  2. Serves as a role model for others and possesses excellent interpersonal skills.
  3. Consistently demonstrates good work habits and dedication to his or her job.
  4. Performs job duties in a timely, efficient manner.
  5. Communicates effectively with other employees.
  6. Is viewed by colleagues as having preformed some noteworthy service to the College.

Nomination Process

Nominations may be submitted by any employee in the College of Public Health, including support staff, managers, administrators, or faculty.  Nominations must be submitted electronically using the form below.  Letters in support of nominations are strongly encouraged and can be sent with the normination form or submitted separately to the Assistant Dean for Finance and Administration (253 Cunz Hall - aflorentine@cph.osu.edu).  Only one nomination form needs to be submitted.  Nominations can be submitted at any time.  The Selection Committee will meet during the Spring and all nominations received by April 30 will be considered during the Spring meeting.  Nominations received after that time will be held until the following year.

Selection Committee and Process

During Winter, each division will elect one faculty member to serve on the Selection Committee.  In addition, the staff member who received the award the previous year will also serve on the committee.  During Spring, the Selection Committee will meet and select the staff person which will receive the award.  The award will be announced at the annual staff appreciation event which is typically held during the summer.  A cash award will be given to the recipient.  Ann Florentine, Assistant Dean for Finance and Administration, will staff the committee and is, therefore, not eligible for the award.
