3.2 Change of advisor assignment

Students who wish to change their advisor assignments may complete the Change in Faculty Advisor Assignment found on the CPH forms page. The form will be routed to the proposed advisor and division chair. If approved, OAPSS will notify the student and their initial advisor of the decision.
